Fix description tables in documentation

This fixes the display of complex descriptions where properties have
blank node values, and makes the output more deterministic.  Most
obvious in the description of params:EnvelopeControls.

Document all URI defines

This makes headers somewhat redundant and too wide, but it's the only
way I can get Doxygen to make links to the corresponding spec
documentation, because it doesn't linkify URIs in strings.
