Fix LV2_ATOM_TUPLE_FOREACH macro.
* using the LV2_ATOM_TUPLE_FOREACH macro throws a compile error
* the 'LV2_Atom_Tuple' has no own size attribute
* '(tuple)->size' should read: '(tuple)->atom.size'
